Urinary Symptoms and Pelvic Floor Physical Therapy

Resolving Urinary Urgency, Hesitancy, Retention, and Incontinence


One of the primary functions of the pelvic floor is to control urination.  The pelvic floor muscles are engage all day to keep urine from escaping, and need to be consciously relaxed in order to release.  When this system is disrupted, several symptoms can commonly occur.

  • Urinary Urgency/Frequency.  It's normal to urinate 4-6 times a day; more than this is considered a symptom.  Tight muscles in the pelvic floor can irritate pelvic nerves, which the brain interprets as the urgent need to use the bathroom.  Your stream should last at least 10 seconds when you use the bathroom if the bladder was full; if it doesn't, you may be experiencing false urgency.

  • Urinary Hesitancy.  When it's difficult to start a stream, or to maintain a steady stream throughout a void, it's called urinary hesitancy.  Often the pelvic floor muscles are unable to relax completely to allow a normal flow of urine.  This is often paired with urinary urgency, so you have the urgent need to go but are often unable to start a stream immediately when you do reach a bathroom.

  • Urinary Retention.  When the bladder is unable to completely empty and residual urine remains in the bladder, it's known as urinary retention.  This can cause an uncomfortable feeling of never being able to fully empty the bladder.  It's also often the underlying cause of frequent or recurring UTIs.


Often the pelvic floor is tight and contains trigger points, or painful knots in the pelvic floor muscles.  These trigger points can irritate the nerves running through the pelvic floor and trigger a feeling of urinary urgency.  This also makes it difficult for the muscles to relax to allow a void, which causes urinary hesitancy or the inability to completely empty the bladder.  

Pelvic floor physical therapy is the most proven way to resolve urinary symptoms.  By eliminating trigger points within the pelvic floor and ensuring the muscles are functioning properly, pelvic PT can resolve these urinary symptoms.

We believe full-length sessions are crucial to success with pelvic PT, so all appointments at PelvicSanity are an hour in length and spent entirely with a pelvic floor specialist. You’ll never be hooked up to electrodes, see an aide instead of a physical therapist, or be told to ‘just do your Kegels.’  We create a positive, calm environment for healing in private treatment rooms.

We also believe the pelvic floor can’t be treated in isolation, so your treatment will address both the pelvic floor and the external muscles that connect and influence the entire pelvis for optimal results. 


Your initial evaluation will be an hour in length, and have three major components:

  • Consultation: Most of our patients have been to multiple doctors, but have never really felt listened to in their appointments.  In your initial evaluation you’ll have a chance to go through your medical history with your physical therapist.  We’ll listen to you and ask about your personal goals for physical therapy.

  • Assessment:  Next your physical therapist will do a complete assessment.  They will look at your posture, hip and pelvic alignment, fascia, scar tissue, and anything else influencing your pelvis.  Then they will do an internal evaluation, looking for trigger points and dysfunction within the pelvic floor muscles.

  • Treatment:  We also want to know how your body responds to treatment, so your initial evaluation also includes targeted treatment.  You’ll also leave with ‘homework’ that you can be doing in between your visits to build on the improvements with physical therapy.
