No — pelvic floor physical therapy should not be painful. While mild discomfort can occur when working with sensitive tissue, true pain can worsen nervous system sensitization and slow healing. High-quality pelvic PT adapts treatment to your body.
Introduction
It’s one of the most common questions we hear:
“Is this supposed to hurt?”
Many patients assume “no pain, no gain” applies to pelvic therapy. It does not.
In this article, we’ll explain what’s normal during pelvic floor physical therapy, what’s not, and how to advocate for yourself.

Why Pelvic Floor Tissue Can Be Sensitive
Pelvic floor dysfunction often involves:
Decreased blood flow
Increased inflammation
Nervous system hypersensitivity
Central sensitization
We call this the dysfunction-inflammation-pain (DIP) cycle.
The tissue may be sensitive — but sensitivity does not justify pain.
Discomfort vs. Pain: What’s the Difference?
Mild discomfort:
Feels tender but tolerable
Does not create fear
Improves during treatment
Pain:
Increases as treatment continues
Feels sharp, alarming, or overwhelming
Persists after the session
If discomfort does not ease, treatment should change.

Why “No Pain, No Gain” Does NOT Apply to Pelvic PT
Chronic pelvic pain alters how your brain processes signals.
Your pain tolerance may be high — but your pain threshold is often lower.
Pushing through pain can:
Increase nervous system alarm
Reinforce central sensitization
Slow recovery
Pelvic PT should calm the system — not inflame it.
What to Do If Pelvic Therapy Hurts
You can say:
“Can we pause for a moment?”
“This isn’t subsiding — it’s getting worse.”
“Is there another technique we could try?”
If the therapist cannot adapt treatment, you are allowed to seek another provider.
