Recovery from pudendal nerve pain depends on the severity of the issue and how long symptoms have been present. While the internet often presents a doom-and-gloom outlook, most cases involve nerve irritation—not permanent damage—and can improve significantly with the right pelvic floor therapy.


Some patients see relief within weeks, while others require longer-term, structured care.



Introduction


Anything involving nerves can feel overwhelming.


If you’ve searched “pudendal nerve damage recovery time,” you’ve likely encountered alarming language online—stories suggesting permanent damage, lifelong pain, or hopeless outcomes.


The truth is far more nuanced.


Not all pudendal nerve issues are the same. In fact, at PelvicSanity, we group these conditions into three categories. Understanding which category you fall into makes an enormous difference in predicting recovery.

Understanding the Three Types of Pudendal Nerve Conditions


Before discussing recovery timelines, it’s essential to clarify what may actually be happening.


1️⃣ Pudendal Nerve Irritation (Most Common)


This is by far the most common scenario.


The pudendal nerve is one of the primary nerves running through the pelvic floor. When surrounding pelvic muscles become tight or overactive, they can irritate this nerve.


Symptoms may include:

  • Deep aching pain

  • Sharp or stabbing sensations

  • A feeling of pressure or “sitting on a golf ball”

  • Burning discomfort

  • Urinary urgency or frequency

  • Pain with intercourse


This is similar to sciatica, where tight back muscles irritate the sciatic nerve and cause pain down the leg.


In this situation, there is nothing structurally wrong with the nerve itself. The nerve is simply being irritated by a tight muscle.


The solution is to relieve that irritation. When the muscle tension is reduced, symptoms can improve dramatically—and often relatively quickly.



2️⃣ Pudendal Neuralgia


Pudendal neuralgia typically begins as prolonged nerve irritation.


When irritation persists for months or years, the nerve can become sensitized. It may begin generating pain signals on its own, even after the original muscular trigger has lessened.


Importantly, this does not mean the nerve is damaged.


It means the nervous system has become upregulated.


Muscles change quickly. Nerves take longer.


For patients with neuralgia, treatment involves:

  • Ensuring the muscle is no longer irritating the nerve

  • Giving the nerve time and space to recover

  • Calming the overall nervous system


This can be a months-to-years process depending on severity.


A skilled pelvic floor therapist will often collaborate with a pain management physician to help reduce symptoms during this time and improve quality of life as quickly as possible. The goal is not just waiting it out—it’s actively guiding the nervous system toward regulation.


This diagnosis is complex and requires patience. But improvement is possible.



3️⃣ True Pudendal Nerve Entrapment (Rare)


True pudendal nerve entrapment is uncommon.


It typically occurs after major pelvic trauma such as:

  • Severe car accidents

  • Major pelvic surgeries

  • Significant childbirth trauma

  • High-impact injury


Some research estimates true pudendal nerve entrapment accounts for less than 1% of cases.


In these instances, medical management is essential. Treatment often includes:

  • Image-guided nerve blocks

  • Pain management support

  • In some cases, surgical decompression


Pelvic floor therapy remains an important part of care—but it works alongside medical intervention.


Entrapment is not the typical presentation, even though it is often what people fear most.

What Is the Typical Timeline for Pudendal Nerve Pain With Pelvic Floor Therapy?


Recovery timelines vary widely depending on:

  • How long symptoms have been present

  • Whether the issue is irritation or neuralgia

  • Nervous system sensitivity

  • Coexisting low back or hip dysfunction

  • Overall health and inflammation


Recent Pudendal Nerve Irritation


If symptoms have only been present for a few weeks, recovery can be relatively fast.


A pelvic floor therapist will:

  • Identify and relax the muscle compressing the nerve

  • Improve hip and spinal mobility

  • Correct breathing and core dysfunction

  • Address the root cause of muscle tension


Many patients notice significant improvement within the first 3–5 visits.


Depending on underlying contributors, therapy may continue for 3–6 months to ensure stability and prevent recurrence.


Long-Standing Pudendal Neuralgia


If symptoms have been present for years, recovery generally takes longer.


Even after muscular irritation is resolved, the nerve itself may remain hypersensitive.


In these cases:

  • Improvement is gradual

  • Progress may feel nonlinear

  • Treatment often extends over several months to a year or more


That does not mean you are stuck.


It means nerves need time to reset.


With consistent care, many patients see dramatic functional improvement—even after long-standing symptoms.



How to Find a Great Pelvic Floor Therapist for Pudendal Neuralgia or Nerve Damage


Not all pelvic floor therapy is equipped to treat complex nerve conditions.


When looking for a therapist, you want to find:

  • A specialty pelvic health clinic—not a general orthopedic clinic that “also does pelvic floor”

  • A provider experienced with chronic pelvic pain

  • Someone who regularly treats complex cases

  • A therapist who works hands-on and one-on-one for the entire visit
